Sanctuary Health is currently looking for an experienced and dedicated Band 5 Community Nurse Practitioner (RMN) to work full-time remotely from home covering the Wiltshire area within a Specialist Veterans Transition and Intervention Team. The ideal candidate will possess highly developed specialist knowledge of mental health nursing and be registered with the NMC.
The pay rate for this locum Community Nurse Practitioner job is £20 per hour (Assignment rate) and is an initial 3-month contract.
Main duties:
- Initial front line assessments, via an online digital platform, to identify any unmet needs, signpost to other services and follow up assessments.
- They will need to use RiO to update healthcare records, risk assessment, diary etc.
Requirements of the Community Nurse Practitioner
- You will need to be a registered practitioner (Mental health nurse, social worker or occupational therapist) with either NMC or HCPC registration.
- Previous community experience.
