Sanctuary Health are currently looking for an experienced and dedicated Band 6 Community Mental Health Nurse to fill a full-time role based in South Westminster. The ideal candidates will possess highly developed specialist knowledge of Mental Health Nursing and be registered with the NMC.
The pay rate for this locum Community Mental Health Nurse job is £26 - £27 per hour (assignment rate) and is an initial 3-month contract.
Main Duties:
- Be accountable for the care and support (under CPA and LPC) of a defined caseload of adults of working age. Assess health and social care needs (including accommodation, finances, and support in accessing education, training or employment) and negotiate care plans with service users and their carers, remaining objective when working with diagnostic uncertainty.
- Deliver/ administer prescribed depot medication to patients both in their own homes or in the clinic as needed - and to assertively follow up on those patients on the caseload who do not attend for their depot.
- Prioritise time and workload, in a flexible manner to meet community caseload responsibilities, participate in meetings and effectively develop personal skills knowledge and experience through supervision and training.
- Undertake assessments of need (in compliance with the Care Act 2014)
- Undertake risk assessments, according to Trust policy.
- Ensure delivery of care plans in partnership with multi-disciplinary colleagues and a range of health and social care agencies, within the CPA framework.
- Engage with service users and carers assertively and offer care/interventions within the least restrictive setting, in normal community environments.
Requirements of the Community Mental Health Nurse:
- Degree or equivalent within Mental Health Nursing (RMN trained)
- NMC registration and a valid live pin
- Must have recent UK Mental Health Nursing experience
