Sanctuary Social Care is currently looking for an experienced and dedicated Community Care Worker to work full-time within a Hospital to Home Team based in Derby. The ideal candidate will possess highly developed specialist knowledge of social work practice in Children's Services and be registered with Social Work England.
The pay rate for this locum Community Care Worker job is £14.40 per hour (Assignment rate) and is an initial 3-month contract.
Summary:
- As postholder of this role you will work within the discharge to assess model working with customers within the hospital and the pathways.
- You will be working collaboratively with the Home First and Occupational therapy team and closely with Carelink & Telecare, LAC, Locality teams and Brokerage.
- Complete assessments in all 3 pathways with the aim to returning customers to their homes.
Requirements of the Community Care Worker:
- Degree or equivalent in social work
- Current Social Work England registration
- Must be a car driver
- Must be responsive and able to work on your own initiative
- Have some experience of Assessing and Reviewing customers, with knowledge of equipment and have a re-ablement ethos.
- You will be required to attend Multi-Disciplinary meetings and possess excellent communication, IT and organisational skills.
