Pay rate (Assignment rate): £250 - £320 per day

Contract Length: 3-Months

Shift Pattern: Full time, 37.5 hours per week. Monday - Friday.

Duties:

  • Manage referrals for complex packages of care by assessing and planning care for service users referred
  • Analyse and interpret clinical information and medical evidence and produce an assessment report and proposed care plan in a professional and concise manner
  • Liaise with registered managers or attend MDT/staff meetings, to present assessment outcomes and clinical information to inform decisions around suitability of proposed placements
  • Manage the expectations of service users/families and other health professionals, who may have an unrealistic idea of timescales
  • Liaise with care coordinators/care managers and keep them informed throughout each stage of the referral and assessment process
  • Contribute to the professional nursing knowledge within the organisation
  • Work well as part of a multidisciplinary team and build positive working relationships with colleagues
  • Apply professional skills and manage own professional competence and accountability, in accordance with the NMC Code of Conduct
  • Confidentiality/data protection regarding all personal information and activity must be maintained at all times (both in and out of working hours) in accordance with professional codes of conduct and relevant legislation such as the Data Protection Act (1998)
  • Promote the concepts of equality of opportunity and managing diversity for both patients and other members of staff
  • Be responsible for the personal safety and well being of others in the course of work
  • Adhere to company policy and procedures on health and safety, with a particular emphasis on infection control in regard to food handling and hand hygiene
  • Adhere to the company’s risk management procedures including the Incident Reporting and Complaints Procedures

Requirements for the CHC Nurse Assessor role:

  • Know and comply with responsibilities under the Health and Safety at Work Act, 1974
  • Be aware of, and comply with their roles and responsibilities in relation to safeguarding vulnerable adults
  • First level registered nurse with the NMC (RMN or RNLD), would consider RGN is able to evidence relevant experience in mental health and learning disabilities
  • Experience of working within the clinical areas of learning disability and mental health
  • Experience of undertaking assessments of complex needs
  • Experience of case management
  • Evidence of strong clinical leadership resulting in high clinical standards
  • Excellent interpersonal and communication skills
  • Strong influencing and negotiation skills
  • Ability to cope with complex organisational issues and able to relate to a wide spectrum of professionals
  • Able to assess priorities and make decisions, quick to grasp a point, able to “think on feet”
  • Knowledge and understanding of key NHS and independent sector issues, including national and local policy drivers
  • Ability to communicate effectively, verbally and in writing; good report writing skills
  • Evidence of continuing professional development (CPD)
  • Committed to raising the standards of the organisation
