Sanctuary Health are currently looking for an experienced and dedicated Band 6 Care Coordinator (Community Psychiatric Nurse) to work full-time within a Community Mental Health Team based in Wandsworth. The ideal candidates will possess highly developed specialist knowledge of Mental Health Nursing and be registered with the NMC.
The pay rate for this Care Coordinator job is £28.53 per hour (LTD Company Equivalent) and is an ongoing contract.
Job Summary:
- To be responsible for the, management of a defined caseload with those suffering from a severe and enduring mental illness.
- To focus on relapse prevention, recovery, the assessment of the effects of the illness on the individual and others, planning and implementing appropriate interventions and evaluating their efficacy.
- To work as a Care Coordinator within the multidisciplinary team in association with members of the Primary Health Care, plus other agencies in order to offer a range of services designed to promote, treat and maintain mental health within the community.
- To implement and/or participate in crisis intervention measures with the client/family or in conjunction with other agencies.
- To provide and promote high standards of care for all patients in the community.
- To promote the concept and principles of a recovery focussed approach.
- To help the client retain or regain mainstream education or work.
- To actively work and support carers.
- To work alongside colleagues in home treatment team,liaison psychiatry and other community mental health teams.
- To develop good working relationships with referrers.
- To be responsible for maintaining up to date care plans, risk assessments alongside other Key performance and CQUIN indicators, CPA and relevant client documentation and be responsible for the major part of implementation of the care plan as CPA care co-ordinator.
Requirements of the Care Coordinator:
- Degree or equivalent within Mental Health Nursing (RMN trained)
- NMC registration and a valid live pin
- Recent UK Nursing experience
