Sanctuary Health is currently looking for a Band 5/6 Community Nurse to fill a full-time role in Kirkby in Ashfield. The ideal candidates will possess highly developed specialist knowledge of Mental Health Nursing and be registered with the NMC.

The pay rate for this locum Community Nurse job is £21 to £33 per hour (LTD Company Equivalent) depending on shift and is an initial 3-month contract (view to being extended).

Duties/Responsibilities:

  • To provide high quality holistic evidence based care to patients / service users that is responsive, effective and efficient.
  • To facilitate and support timely discharge and prevent inappropriate admissions by supporting patients / service user to remain as independent as possible and maintain or improve their level of function and quality of life.
  • To support the re-design of Adult Community Health Services within Nottingham to achieve a model of wrap around care that is responsive to the needs of the patients/service users.
  • To be pro-active in delivering anticipatory and responsive care, promoting independence, health life style, reduction in health inequalities to improve the overall quality of life in the local population.
  • To offer a comprehensive nursing service to the resident population of Nottingham West, providing a multi-disciplinary and multi-agency approach to maximise patient function, well-being and self-care planning.

Requirements of the Community Nurse:

  • Valid NMC registration
  • Degree or equivalent in Nursing
  • Recent UK Nursing experience
  • Community nursing experience
  • Must have own transport or ability to travel
