Pay Rate: £34 per hour (assignment rate)

Main Duties: 

  • Provide a professional social work service to residents, carers, and communities in Croydon, adhering to national legislation, guidance, and local policies and procedures.
  • Work within a place-based, skill-mixed social care team, collaborating through GP huddles, the Integrated Care Network (ICN), and with the Alliance and community partners.
  • Promote residents' independence, wellbeing, and social engagement, safeguarding vulnerable adults by delivering services at the right time, to the right people, and in the right place.
  • Manage a complex caseload, ensuring all assessments comply with the Care Act, are proportionate to need, and demonstrate resident-led, positive outcomes. Implement preventive measures to avoid transitions to high-cost services, and monitor and review as necessary.
  • Practice as a Best Interests Assessor (BIA) and Safeguarding Adults Manager (SAM).
  • Supervise staff and assist the Team Manager with staff development, best practices, and continuous professional development through reflection.
  • Deputise for the Team Manager as required, supporting the operational implementation of cultural and behavioral changes in practice.
  • Undertake projects or participate in initiatives as agreed with line managers.
  • Collaborate with all teams across the division to develop innovative approaches, ensuring resilient service models are in place. Maximise residents' rehabilitation and reablement potential to prevent hospital admissions and ensure timely hospital discharges.

Requirements of the Social Worker:

  • Degree or equivalent in social work
  • Current Social Work England registration
  • Significant frontline experience in Adults Services
