Are you a dedicated and compassionate Social Worker looking for a rewarding opportunity? We are currently seeking an experienced Adult Social Worker to join our Review & Support Team in Kingston upon Hull.

Key Responsibilities:

  • Conducting assessments of service users' needs and creating personalised care plans.
  • Providing support and guidance to service users, their families, and caregivers.
  • Collaborating with other professionals and agencies to ensure holistic support for service users.
  • Reviewing and updating care plans regularly to reflect changing needs and circumstances.
  • Advocating for service users to ensure their rights and preferences are respected.
  • Working within legal and regulatory frameworks to safeguard vulnerable adults.
  • Maintaining accurate records and documentation of interactions and interventions.

Requirements:

  • Qualified Social Worker with a relevant degree and Social Work England registration.
  • Proven experience in adult social work, preferably within a similar setting.
  • Strong assessment and analytical skills, with the ability to identify needs and risks effectively.
  • Excellent communication and interpersonal skills, with the ability to build rapport and trust.
  • Ability to work effectively within a multidisciplinary team and collaborate with external partners.
  • Sound knowledge of relevant legislation, policies, and procedures related to adult social care.
  • Commitment to continuous professional development and adherence to ethical standards.
