Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for an Adult Social Worker within the Integrated Neighbourhood Team to work full time based in Harrow.

The salary for this permanent Social Worker job is up to £42,459 per annum.

Main duties:

  • To work in a strength-based way with adults and their families and carers, including those who are terminally ill. This includes working with multi-disciplinary colleagues, providing advice and information, working with community supports and liaison with other agencies, seeking the help of other professionals as necessary.
  • To have person centred conversations with people and carers in a timely way evaluating the level of risk and an evidence-based analysis of need whilst promoting independence in accordance with agreed criteria and divisional policies.
  • To be alert for instances when people may be at risk and to ensure safeguarding adult concerns are made as appropriate within required timescales.
  • To assess for and provide equipment and minor adaptations to promote independence, and to monitor these as necessary, seeking advice where appropriate.
  • Where services are required, to make recommendations for innovative, cost effective and quality care packages to promote independence, to obtain authorisation from the relevant budget holder and arrange the provision of services.
  • To maintain and take responsibility for effectively managing a personal caseload including complex cases in consultation with the Team Manager.
  • To work as a member of a multi-disciplinary team liaising with other professionals including health and the voluntary sector ensuring that there is the fullest co-operation with other team members, to provide an easily accessible service geared to meetings people’s needs.
  • To ensure, along with other colleagues in other parts of the department, that people and their carers are involved in all decisions and advocates are recommended where appropriate.
  • To liaise with staff throughout social services as well as other departments of the local authority and to liaise with statutory, voluntary, and independent organisations to ensure that working relationships are maintained, developed, and strengthened.
  • To prepare and present reports in a clear and concise and logical way and to a high standard, which contribute to multi-agency and multiple disciplinary conferences and meetings and court hearings and attend these forums. Also, to ensure that actions from the conference/meeting/court hearing are completed within agreed timescales.
  • To undertake any other duties that may be required commensurate with the general level of responsibility of the post.

Requirements of this Social Worker role:

  • A social Work Qualification or equivalent.
  • Understanding of relevant legislation.
  • Registered or eligible for registration.
