The successful candidate will work closely with the District Nurse Team Lead to manage a defined caseload within a specific geographical area. This role involves supporting, leading, and managing a team of nurses to deliver high-quality patient care. As a Specialist Practitioner, you will provide clinical advice to the District Nursing Service and multidisciplinary community teams, ensuring effective inter-professional collaboration with other specialist nurses. Main Duties and Responsibilities: Oversee the assessment, development, implementation, and monitoring of health and social care, ensuring the highest standards of care and service delivery in home and clinical settings. Assess, prescribe, and evaluate treatments independently or in partnership with other prescribers. Collaborate with the District Nurse Team Lead to meet team and service objectives. Act as an assessor and supervisor for students, and preceptor for newly qualified staff. Establish and maintain effective working relationships with patients, carers, and the Health and Social Care Team, including statutory and voluntary agencies.
