Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work full-time in Swindon, within the Transitions Team. The ideal candidate will possess highly developed specialist knowledge of social work practice in Adults' Services and be registered with Social Work England.
The salary for this permanent Social Worker job is £31,369 - £32,640 per annum.
Duties of the Social Worker:
Work closely with young people between the ages of 14 years and 25 years who may have a learning disability, physical disability or mental health needs and support them to prepare to enter adulthood and develop their independence as much as possible.
To get to know a young person and their family over a longer period.
Completing the Care Act Assessment in preparation for the young person turning 18 years old.
Working in a multi-disciplinary framework with education, health, children services and supported employment services amongst others before transitioning to Adult Social Care.
Managing a caseload that may include capacity assessments, completing DOLS applications and attending MDT meetings.
Requirements of the Social Worker:
Degree or equivalent in social work
Current Social Work England registration
Significant senior level frontline experience within Adults' Services
Must have experience of and a passion to work with young adults who may have a learning disability, physical disability or mental health needs and are committed to providing a person centred and strengths-based approach to their work.
Must be fluent in the English language (as a requirement of Part 7 of the Immigration Act - for the effective performance of a customer-facing role).
