Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker/Safeguarding Lead to work full-time in Blackheath. The ideal candidate will possess highly developed specialist knowledge of social work practice in Adults' Services and be registered with Social Work England.
The salary for this Social Worker/Safeguarding Lead job is £34,000 - £38,000 per annum and is on a permanent basis.
Duties:
Work as a care coordinator or associate worker (where applicable) with individuals who have complex needs due to a Brain Injury or neurological condition
Manage the service provision for a defined caseload of patients
Ensure appropriate risk assessments are complete
Delegate the delivery of specific interventions to nurses/rehab Assistant and Therapists
Liaise closely with other agencies at the point of discharge
Undertake financial assessments for all patients
Ensure any DoLs applications are completed and submitted
Ensure that all relevant data is entered onto THG information systems
Use the results of and participate in clinical audit and research
Complete training for staff on safeguarding and MCA in induction
Requirements of the Social Worker/Safeguarding Lead:
Degree or equivalent in social work
Current Social Work England registration
Significant senior level frontline experience within Adults' Services
Minimum of 2 years experience within a neuro-rehab/brain injury setting
