Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker/Pathways Advisor to work full-time in Calderdale, within the Looked After Children and Care Leavers Team. The ideal candidate will possess highly developed specialist knowledge of social work practice in Children's Services and be registered with Social Work England.

The salary for this permanent Social Worker/Pathways Advisor job is £28,180 - £29,169 per annum. This is an unqualified role working with care leavers age 18 - 25.

Duties of the Social Worker/Pathways Advisor:

  • Ensure adherence to the company's equalities and diversity policies.

  • This role will hold a case load of children in care aged 16 to 18 and therefore deliver the statutory requirements for the cohort. In addition, it will hold a case load of 18 to 25-year olds who are eligible care leavers.

  • Establish positive relationships with young people and their families, encouraging active engagement and participation in all aspects of the process

  • Undertake comprehensive assessments of young people using an appropriate assessment process including the analysis and assessment of risk within a required time frame

  • Manage and maintain a caseload of young people some of which will be between the ages of 16 to 18 and some will be 18 to 25. The post will be responsible for ensuring their needs are assessed and effective interventions and programmes are in place to meet need

  • Lead in the development and implementation of agreed intervention plans for young people and their families, taking into consideration complexity of need, level of risk, age and ability including pathway plans.

  • Oversee and co-ordinate delivery of interventions through a variety of engagement strategies.

  • Undertake regular reviews of intervention and pathway plans and identify progress and gaps or changes required to assist transition

  • Work with a range of partners to deliver effective services and negotiate and agree working Partnership Agreements.

  • To act as a thematic lead for a specific area of work within the team and contribute your specific skills & knowledge.

Requirements of the Social Worker/Pathways Advisor:

  • Degree or equivalent in social work

  • Current Social Work England registration

  • Significant senior level frontline experience within Children's Services
