What you will do:
- Serve as the primary contact point for referrals into Adult Social Care
- Screen and assess referrals, ensuring appropriate and timely responses
- Signpost to relevant services and provide Information, Advice, and Assistance
- Work collaboratively with the Early Response team to coordinate service delivery
- Maintain accurate records and documentation in line with organisational requirements
- Engage with a range of stakeholders, building effective relationships to enhance service provision
