Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work full-time in Watford, within the Older People's Team. The ideal candidate will possess highly developed specialist knowledge of social work practice in Adults' Services and be registered with Social Work England.
The pay rate for this locum Social Worker job is £26 per hour (LTD Company Equivalent) and is an initial 3-month contract.
A team of experienced SW, OT and Community Care Officers who provide an assessment and care management function to older people who live in Watford (over 65 years although younger people with early onset dementia come under our service).
Duties of the Social Worker:
- Dealing with referrals for a range of complex work
- Carrying out assessments for service user who require residential or nursing home placements
- Working with service users who are coming to terms with significant life change
- Carrying out initial assessments of older adult's care needs
- Presenting complex cases to the Multi-Disciplinary Team
- Completing Risk Assessments
- Undertaking Mental Health Capacity Assessments
- Participating in Best Interest meetings
- Undertaking Care Act eligibility and carer assessments
- Setting up of direct payments
- Arranging and reviewing care plans
- Keeping up to date case notes
- Making referrals for Extra Care Housing Tenancies
- Participating in professional meeting involving complex meetings
- Undertaking reviews with registered care homes
- Participating in the team's duty rota
- Safeguarding Older People
- Protection Planning for Older People
- Carrying out community care assessments
- Responding to crisis situations
Requirements of the Social Worker:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant senior level frontline experience within Adults' Services
- Must have passion for working with older people and interest in working with our partner agencies to achieve best possible outcomes for service users
