What you will do:
- Review and assess incoming referrals and contacts within the MASH to determine appropriate pathways and responses.
- Conduct threshold conversations and home visits to engage children, families, and carers, ensuring their safety and support.
- Collaborate closely with team members, including social workers, health professionals, education specialists, and police officers.
- Develop and maintain comprehensive records, reports, and documentation to facilitate case management.
- Provide informed recommendations and case presentations to management teams for effective service delivery.
- Participate in multi-disciplinary meetings, interfacing with various agencies to coordinate efforts and enhance service outcomes.
