Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work full-time in Newham, within the Mental Health Team. The ideal candidate will possess highly developed specialist knowledge of social work practice in Adults' Services and be registered with Social Work England.
The pay rate for this locum Social Worker job is £29 per hour (LTD Company Equivalent) and is an initial 3-month contract.
Duties of the Social Worker:
- To undertake assertive outreach in order to engage clients who have previously been lost to follow up, do not wish to engage with services and for whom there is concern.
- To visit clients at home, day centres, work and recreational centres to engage and encourage services participation in their care plan.
- To provide practical assistance as a means of engaging with and supporting users of the service.
- Provide information to help service users make informed choices about opportunities and service available to them.
- To involve families and carers in treatment plans where appropriate.
- To participate in a duty service with other members of the team.
- To participate in multidisciplinary assessments and care planning and carry out care management responsibilities for those with complex needs.
- To participate fully in the development and maintenance of good liaison, coordination and co-operation with the local community, community groups, other agencies and on-statutory organisations, including outreach work as appropriate.
- To attend and participate positively in team meetings.
Requirements of the Social Worker:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant senior level frontline experience within Adults' Services
- Previous experience working within a Mental Health Team is essential
