Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for a Social Worker within the Mental Health Team to work Full Time based in Kingston.
The salary for this permanent Social Worker job is up to £46,510 per annum.
Main duties:
- To ensure compliance with the professional code of conduct and fulfil obligations under the relevant legislation.
- To be fully responsible for managing a caseload of customers and carers with complex needs.
- To provide a professional social work intervention aimed at achieving specific outcomes in order to improve the customers wellbeing, independence & recovery.
- To effectively work in collaboration with health colleagues from Mental Health Trusts in order to provide an effective integrated health and social care service.
- Promote user empowerment and anti-discriminatory practice.
- To provide practice assessor and/or practice educator role for AMHP, ASYE, Social Work Students.
- To develop and maintain effective working relationships with all partners (both internal and external) in order to ensure that the team can deliver a co-ordinated, timely and appropriate service to the customer.
- To promote and participate in good team working practices and to demonstrate these whenever possible.
- To keep up to date with developments, products and information relating to the service and the rest of the council to ensure that appropriate and timely information is given to customers.
- To ensure proportionate risk assessments for both the customer and the carer associated with any support plan have been carried out.
- To undertake reviews of customers with long-term needs as required and within statutory timelines and carry out reassessments as required.
- To actively and co-operatively as an effective team member, collaborating with and informing colleagues within the team, across the council and with partners.
- To be responsible for accepting referrals, undertaking screening, and carrying out assessment of need of customers.
- To ensure appropriate and timely intervention for customers who present with urgent needs, thus ensuring their immediate wellbeing and safety.
- To facilitate purposeful activity and occupation, meaningful relationships, suitable housing, maximising income to access community, support to recognise personal history and reduce risks around abuse or neglect.
Requirements of this Social Worker role:
- A Social Work Qualification or equivalent.
- Understanding of relevant legislation.
- Relevant Social Work experience.
