What you will do:
As a Social Worker within the Mental Health Team, your main responsibilities will include:
- Conducting thorough s9 assessments under the Care Act.
- Facilitating s27 reviews to ensure the ongoing safety and well-being of service users.
- Providing essential s117 aftercare to service users transitioning from mental health services.
- Working closely with vulnerable adults to ensure compliance with MCA 2005, MHA 83/07, and Equality Act 2010 frameworks.
- Developing robust care plans and utilising strengths-based and person-centred approaches.
- Collaborating with multidisciplinary teams, carers, and external agencies to deliver the best outcomes.
- Keeping detailed records and preparing reports for relevant stakeholders.
