What you will do:
- Conduct comprehensive assessments of individuals with mental health conditions, evaluating their physical, emotional, and social needs.
- Develop, implement, and review care plans in collaboration with service users and their families.
- Support individuals to access relevant mental health services, resources, and interventions.
- Provide case management and advocacy to ensure individuals receive necessary services and support.
- Liaise with healthcare providers, housing, and community agencies to coordinate care and services.
- Participate in team meetings, supervision, and continuing professional development activities.
- Maintain accurate and timely records of assessments, care plans, and interactions with service users, adhering to safeguarding protocols and confidentiality guidelines.
- Provide expert social work support in crisis situations, ensuring effective risk management and decision-making.
