Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work full-time within a Learning Disabilities CHC Project. This role is based in Cambridgeshire but the specific working location is flexible (in Cambridgeshire).
The ideal candidate will possess highly developed specialist knowledge of social work practice in adults' services and be registered with the Social Work England.
The pay rate for this locum Social Worker job is £28 per hour (LTD Company Equivalent) and is an initial 6-month contract.
Duties of the Social Worker:
To review and assess care packages that fall within the scope of CHC guidance to ensure that Adults with learning disabilities continue to have access to a CHC service which offers an appropriate level of care
To ensure residents of Cambridgeshire (predominantly people with learning disabilities) continue to have access to a seamless Continuing Health Care service which offers the appropriate level of care at the optimum cost to the public
Ensure the approximately 700 care packages have been correctly assessed and finance apportioned as per care package
Requirements of the Social Worker:
Degree or equivalent in social work
Current Social Work England registration
Significant senior level frontline experience within adults' servicesÂ
Essential Qualifications/ Skills
Excellent case management skills
Knowledge of the CHC process
Experience within learning disabilities, older people or hospital discharge
Professional Social Work qualification
