What you will do:
- Work within a multidisciplinary community social care team to provide high-quality services for individuals with complex needs.
- Conduct assessments, support planning, and reviews to ensure individuals receive the necessary long-term support.
- Carry out Mental Capacity Assessments and contribute to shaping personalised care plans.
- Manage an allocated caseload within agreed frameworks to meet performance and quality standards.
- Ensure that service users, carers, and families are engaged in an effective, personalised way to meet their needs.
- Support individuals to exercise choice and control to achieve their defined outcomes.
