Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work within an Adults Integrated Discharge Team based in Manchester. The ideal candidate will possess highly developed specialist knowledge of Social Work practice in Adult Services and be registered with the Social Work England.
The pay rate for this locum Social Worker job is £25.02 per hour (LTD Company Equivalent).
About the role:
- Strength Based assessments/ Person centred support Plans
- Mental Capacity Assessments-Best Interest meetings/ Discharge planning meetings
- Case Recording-In Liquid Logic and the I.T health System-EPR
- Safeguarding Investigations, Knowledge and understanding of legislation, Policy and Procedure.
- Maintaining Patient flow and facilitating discharges is vital to ensure, Citizen's Independence is maintained.
Requirements of the Social Worker:
- Degree or equivalent in Social Work
- Current Social Work England registration
- Significant frontline experience within Adult Services
- Desired to have hospital Social work experience and the knowledge and experience associated with the fast pace of work within an acute hospital setting.
- The I.T. system in operation is Liquid Logic, experience is preferential.
- Required to work 35 hours weekly Mon-Fri
