Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work full-time in Swindon, within the Hospital Team. The ideal candidate will possess highly developed specialist knowledge of social work practice in Adults' Services and be registered with Social Work England.
The salary for this permanent Social Worker job is £31,369 - £32,640 per annum.
The team provide outstanding care planning incorporating the voice of the service user from initial referral, encompassing the views of multi-disciplinary professionals, families/carers and external agencies to provide ideal outcomes and safe discharge for service users. Completing assessments and management of complex discharge pathways, providing the highest standards of care alongside safe and timely discharges.
Requirements of the Social Worker:
Degree or equivalent in social work
Current Social Work England registration
Significant senior level frontline experience within Adults' Services
