Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work full-time in a Hospital Team based in Sutton. The ideal candidate will possess highly developed specialist knowledge of social work practice in adults' services and be registered with the HCPC.
The pay rate for this locum Social Worker job is £30 per hour (LTD Company Equivalent) and is an initial 3-month contract.
About the role:
- Complete short assessments in crisis to ensure appropriate support is provided in a timely manner to both prevent hospital admission, and in order to facilitate hospital discharge.
- As part of this role, Social Workers will also be required to complete Mental Capacity Assessments and Best Interest Decisions, contribute to NHS Continuing Healthcare assessments, support individuals to address issues relating to the habitability of their property, and make applications to Court of Protection regarding deputyship for finances.
Requirements of the Social Worker:
- Degree or equivalent in social work
- Current HCPC registration
- Significant senior level frontline experience within adults' services
- Hospital experience
