Join our Lambeth Hospital Team as a Social Worker! Based at the renowned St Thomas’ Hospital, we seek a motivated and skilled Social Worker to embark on a rewarding locum position, initially for three months with a chance to extend. The role offers an impressive hourly salary of £32. You will work Monday to Friday, clocking in 35 hours a week with the bonus of hybrid working—three days in the office and two days at home.

What you will do:
- Collaborate with the Lambeth Hospital Social Work Team to provide strength-based, preventative support.
- Conduct evaluations of existing care packages, considering placements and ensuring safe and efficient patient discharges.
- Identify opportunities for patients to benefit from reablement services, encouraging the enhancement of their daily living skills.
- Engage with multidisciplinary teams (MDTs) and voluntary organisations to deliver a multi-faceted support plan.
- Undertake assessments and reviews with adults over 18, dealing with a variety of physical health disabilities and vulnerabilities.
- Demonstrate a deep understanding of safeguarding procedures to protect and support patients effectively.
