What you will do:

- Advocate for patients’ rights and ensure compliance with the Care Act, Mental Capacity Act, and safeguarding procedures.
- Support individuals in their transition from hospital to community, promoting independence and well-being.
- Respond efficiently to safeguarding issues and contribute to the overall safety of vulnerable adults.
What you’ll need:
- To be a Qualified Social Worker with registration from Social Work England.
- Experience in hospital settings or adult social care.
- Strong understanding of relevant legislation, including the Care Act and Mental Capacity Act.
- Exceptional assessment and communication skills, with the ability to work effectively across multiple agencies.
