Sanctuary Personnel, an innovative and committed recruitment agency is currently looking for a dedicated and experienced Social Worker to work based in Wandsworth, within the Hospital Team.

The pay rate for this locum Social Worker role is up to £30 per hour (Assignment rate) dependent on experience and is a 1-month contract.

Working Hours:

  • Service time: Monday - Saturday, 8am - 8pm service.
  • Front line staff work between 9am - 6pm on Monday - Friday and 9am - 5pm on Saturday.
  • Work pattern: Monday - Friday or Tuesday to Saturday, flexible to discuss work pattern in interview.
  • Hybrid mix - working from home and in the office (health and social care offices)

Main Duties:

  • Work alongside Wandsworth health partners to discharge patients within a timely and safe manner and jointly assess long term care needs once patient is at home.
  • Work with acute hospital trusts within a discharge to assess model (trusted assessor model).
  • Work with enablement – identify reablement goals and review progress when patient is at home with discharge to assess care.
  • Complex case management - lead work liaising with and coordinating partners involved in patients discharge to assess care, ensuring care is strengths based and patient is at the centre of care planning.
  • Duty - screen and scrutinise referrals, liaise with patients and hospitals to ensure patient is discharged in a timely and safe manner.
  • Care Act Assessments, care and support planning, funding requests and mental capacity assessments.
  • Safeguarding - screen to enquiry outcome stage.
  • CHC assessments – Participate in DSTs and subsequent long term accommodation decisions dependent on outcome of funding.
  • Patient contact: complete face to face home visits and visit hospitals to assess patients for complex patients if required. Full PPE provided.

Requirements of the Social Worker:

  • Degree or equivalent in social work.
  • Current Social Work England registration.
  • Recent post-qualifying experience within frontline social work.
