Sanctuary Social Care is currently looking for experienced and dedicated Social Workers to work full-time in Sutton within the Hospital Pathway Team. The ideal candidate will possess highly developed specialist knowledge of social work practice in Adults' Services and be registered with Social Work England.
The pay rate for these locum Social Worker jobs is £28 per hour (Assignment rate) and is an initial 3-month contract.
Duties:
- Undertake Mental Capacity Act assessments
- Undertake Care Act Assessments and Safeguarding assessments
- Carry out a care plan
- Undertake a caseload
- Contribute to discharge the admission of patients to the Hospital
- Attend meetings
Requirements of the Social Workers:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant senior level frontline experience within Adults' Services
- Minimum 2 years post qualifying UK experience
