Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work full-time within the Hospital Integrated Discharge Team based in Bexley. The ideal candidate will possess highly developed specialist knowledge of social work practice in Adults' Services and be registered with Social Work England.
The pay rate for this locum Social Worker job is £28.00 per hour (Assignment rate) and is an initial 3-month contract.
Duties:
- Work with service users and their carers, to promote and maintain their dignity, choice and independence Assist as part of the multidisciplinary team in improving quality of life by, devising re-enablement programmes to maximise independence and assist with accessing resources to provide practical help, where appropriate
- Give advice and information, and signpost people to appropriate provision in the voluntary and independent sectors
- Facilitate the safe discharge of service users from hospital
- Assess the needs of older people and people with physical and learning disabilities or mental health problems, and their carers
- Devise innovative packages of care in consultation with service users, and carers that respond to individual needs and are managed within the available council budget and resources, including the use of Individual Budgets
- Carry out statutory duties, this will include the safeguarding of adults policy and procedures
- Participate in the Social Work emergency duty rota
- Ensure effective and efficient use of time and resources
- Provide reports and necessary documentation
Requirements of the Social Worker:
- Degree or equivalent in social work
- Current Social Work England registration
- Understanding and experience of working with older people, people with disabilities, and long term health conditions and their carers
- Knowledge of social work methods and interventions
- Ability to supervise students
- HCPC registration
- Completed the ASYE award
- Approved Mental Health Professional Accreditation
- Qualified Best Interests Assessor
- Qualified Practice Educator Stage 1/2
