Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work full-time in Doncaster, within the Hospital Discharge Team. The ideal candidate will possess highly developed specialist knowledge of social work practice in adults' services and be registered with the HCPC.
The pay rate for this locum Social Worker job is £27 per hour (LTD Company Equivalent) and is an initial 3-month contract.
Duties include assessments in accordance with the Care Act 2014 and Mental Capacity Act 2005 to facilitate discharge from the acute hospital setting. The team also perform a duty function and offer support and assessment in the Emergency Department to prevent reduce and delay the need for formal support and services. We are based in a multi-disciplinary team with health professionals and other disciplines to work together providing a holistic assessment and level of support for discharge.
Requirements of the Social Worker:
- Degree or equivalent in social work
- Current HCPC registration
- Significant senior level frontline experience within adults' services
- A knowledge of safeguarding, delayed transfers of care is beneficial
