Sanctuary Personnel, a leading and award winning recruitment agency is currently looking for an experienced and dedicated Social Worker to work full-time based in Doncaster, within the Hospital Discharge Team.
The pay rate for this locum Social Worker job is £28 per hour (Assignment rate) and is an initial 3-month contract.
Duties:
- Provide discharge facilitation services within the Integrated Discharge Team based at Doncaster Royal Infirmary or in peoples’ own homes
- Provide a high quality, integrated, effective social work service to adults and carers in accordance with the Care Act 2014 statutory requirements, Mental Capacity Act 2005, local policy, procedure and guidance
- Assessing patients’ needs to ensure a safe, timely discharge from the hospital setting
- Work in a busy environment and skills in organisation prioritising work and a flexible approach are essential along with excellent recording skills meeting GDPR requirements
- Undertaking holistic, strengths based assessments with adults, carers and families to help them make informed choices and decisions, enabling them to clarify and express their needs, manage risk and contribute to service planning
- Provide written and verbal records and reports which are concise, factual and informative based on analysis of evidence which can be used within the legal context and where necessary give evidence in court in relation to care or other proceedings
- Use evidence based practice and a strengths based conversation to identify need and take appropriate action which is outcome focused
- Support discharge from the acute setting to assess in peoples’ own homes for longer term needs or similar settings after discharge
Requirements of the Social Worker:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant senior level frontline experience within Adults’ Services
- Be able to work autonomously using initiative to manage own complex caseload
