Sanctuary Social Care is currently looking for an experienced and dedicated Senior Social Worker to work full-time in Manchester, within the Hospital Discharge Team. The ideal candidate will possess highly developed specialist knowledge of social work practice in Adults' Services and be registered with Social Work England.
The pay rate for this locum Senior Social Worker job is £26.88 per hour (Assignment rate) and is an initial 3-month contract.
The Hospital Discharge to Assess Team (HDTA) team is based within the community and hospital; overseeing hospital discharges on a discharge to assess pathway and responds to urgent/crises situations to enable the citizens to continue living in their own home for as long as possible.
Duties:
- Carry a small, complex case load.
- Supervision of Social Workers in line with Manchester City Council policy.
- Provide practice advice and guidance to Social Workers to ensure work is carried out within a multi-disciplinary framework which meets the needs of citizens and promotes their independence.
- Support the management team with Social Work governance to ensure that systems and processes are adhered to.
- Work in a collaborative way with health colleagues, voluntary sectors and other partners focusing on asset based and strength based
Requirements of the Senior Social Worker:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant senior level frontline experience within Adults' Services
