Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work full-time in Enfield, within the Hospital Discharge Team. The ideal candidate will possess highly developed specialist knowledge of social work practice in Adults' Services and be registered with Social Work England.
The pay rate for this locum Social Worker job is £31.15 per hour (LTD Company Equivalent) and is an initial 3-month contract.
Duties of the Social Worker:
- Undertaking comprehensive social care assessments of needs including Human Right, Mental Capacity Assessment or other appropriate reports in line with adult social care remit, with detailed support plans to facilitate patients' safe discharges.
- To communicate closely and sensitively with family members of service users who are deceased
Requirements of the Social Worker:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant senior level frontline experience within Adults' Services
- To have sound knowledge around adult legislation and the continuing health care process and the legislation that underpins this, including the CHC Checklist, Decision Support Tools, London Health Needs Tool
- Knowledge and Skills on IT systems are essential
- Previous experience working in a hospital team is essential
