Sanctuary Personnel, a leading recruitment company to the private and public sector has a locum vacancy for a Social Worker to work full time based in Barnstaple, within the Hospital Care Team.
The pay rate for this locum Social Worker is £31-£35 per hour (Assignment rate) and is a 3-month contract.
Main Duties:
Join the team made up of experienced social workers and social care assessors who work alongside doctors, nurses, occupational therapists, physiotherapists, mental health services and the wider multidisciplinary team to facilitate appropriate, safe and timely hospital discharges for those referred to the service.
Undertake mental capacity assessments and make subsequent best interest decisions.
Undertake face to face visits to people who are in hospital to determine a discharge plan in conjunction with them, their families/carers and the wider multidisciplinary team. Following discharge from hospital the team complete a care act assessment to determine individuals ongoing eligible care needs and planning how to support these.
Requirements of the Social Worker:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant frontline experience in Social Services
- Have a good working knowledge of safeguarding process and practice as well as having an in-depth knowledge of the Mental Capacity Act
- Due to the team covering a large geographical area the ability to drive and have access to a vehicle for work is essential
