Sanctuary Personnel, an innovative and committed recruitment agency has a job based in Northampton for a Social Worker to work 37 hours per week, within the Discharge to Assess/Community Team.
The pay rate for this locum Social Worker job is £30 - £32 per hour (Assignment rate) and is an initial 3-month contract.
Hybrid working around assessments is possible, however the expectation would be the initial few weeks would require more office presence to allow for training. If remote working - laptop and phone to be collected from the office at the start of the assignment. If office based - full Covid-19 protection measures are in place. Most offices are working on a rota basis, however all offices are set up to be Covid safe. It will be a mixture of remote working but also visiting provisions. Travel throughout the borough is required.
Duties:
- Conduct face to face assessments to those who have been discharged from a hospital and are either in a residential home or their own home (risk assessment and PPE provided)
- Work with the person, their family and the brokerage team to either make the existing placement permanent, move to an alternative provision or return home with a CCP
- The completion of a care act assessment and other supporting pieces of work as required (e.g financial assessment/ mental capacity assessment etc)
Requirements of the Social Worker:
- Social Care Qualified
- Current Social Work England registration
- Already be working in a similar field, or have enough experience like for like that they are able to ‘hit the ground running’
- Be double-vaccinated, and be prepared to provide evidence to include the dates that the vaccine was received
