Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work full-time in Shropshire in a Discharge to Access Team. The ideal candidate will possess highly developed specialist knowledge of social work practice in adults' services and be registered with the HCPC.
The pay rate for this locum Social Worker job is £27.89 per hour (LTD Company Equivalent).
Duties:
Participate in the Admission Avoidance Rota.
Requirements of the Social Worker:
Degree or equivalent in social work
Current HCPC registration
Significant senior level frontline experience within adults' services
Social Workers will need to be flexible, working towards the Discharge2Assess Model, and be trained to undertake MCAs & BIs, experience in working the acute including A&E departments and community hospitals, including Out of County, and within the community with peoples' own homes.
