Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work full-time in Rochdale, within the Community Mental Health Team. The ideal candidate will possess highly developed specialist knowledge of social work practice in adults' services and be registered with the HCPC.

The pay rate for this locum Social Worker job is £25.02 per hour (LTD Company Equivalent) and is an initial 3-month contract.

The Borough-wide Review Team is based within the CMHT at Hanson Corner Middleton and has oversight of individuals in receipt of commissioned support either in the community or in long-term placements; who although complex. are largely settled and require periodic reviews under the Care Programme Approach (CPA) and Care Act frameworks. Some individuals are placed out of Borough, which may require travel and joint working with care coordinators who hold CPA responsibility.

Duties of the Social Worker:

  • Work will undertaking CPA reviews and reviewing eligibility for s117 after-care to ensure that the Local Authority is meeting its funding responsibilities.
  • There may also be occasion to work with long-stay hospital patients to attend CPA meetings, complete Social Circumstance reports and liaison with the integrated commissioning team and provider agencies to facilitate packages of support on discharge, as well as linking in with CMHT active care coordination and Adult Care Services where transfer of case responsibility is deemed appropriate.

Requirements of the Social Worker:

  • Degree or equivalent in social work
  • Current HCPC registration
  • Significant senior level frontline experience within adults' services
  • Must be a car driver
  • Must have a background in adult community mental health, who can work independently is sought to support the Borough-wide Review team in completing Care Act assessments/reviews, promoting the use of assistive technologies and commissioning both managed and cash budgets to improve user outcomes
