Sanctuary Personnel, a leading and award winning recruitment agency is currently looking for an experienced and dedicated Social Worker to work part time or full-time based in Shrewsbury, within the Community Interventions Pathway Team.

The pay rate for this locum Social Worker job is £25 per hour (Assignment rate) and is an initial 3-month contract.

Shift Pattern:

  • Monday – Friday
  • 09:00 am – 17:00 pm

Duties:

  • Working in an identified adult community mental health pathway, with the flexibility of working across pathways, dependent on skill and service need
  • Delivering a high quality clinical service to patients and their families
  • Managing a complex caseload and responsible for clinical and caseload supervision of other team members where identified
  • Be responsible for providing the nursing process to a specialist clinical pathway
  • Delivering nursing interventions, in accordance with an agreed care plan, to an identified group of people experiencing mental health related difficulties
  • Be responsible for working in an identified adult community mental health pathway, with the flexibility of working across pathways, dependent on skill and service need
  • Be responsible for care coordination of an identified caseload
  • Be responsible for undertaking clinical assessments, in accordance with the clinical pathway, in order to establish the person’s needs, goals, risks and strengths in order to generate a formulation based care plan agreed with all involved
  • Be responsible for delivering nursing interventions, in accordance with an agreed care plan, to an identified group of people experiencing mental health related difficulties
  • Be available for consultation, support and clinical supervision to other health professionals working within the Community Mental Health Pathway Service and other areas of the Trust where identified as appropriate
  • Take responsibility for the management of a complex caseload, which includes,  specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans (Care Programme Approach and Single Assessment Process)
  • Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy
  • Administer medication, complying with relevant Trust procedures and NMC Guidance
  • Assess patient's individual care needs, develop, implement and evaluate programmes of care

Requirements of the Social Worker:

  • Community Mental Health Practitioner /Social Worker or Occupational Therapist (Band 6)
  • Driver, mileage paid at 40p per mile
  • Be available for individual supervision with agreed clinical supervisor in line with Trust policies
  • Standard keyboard skills required for inputting data onto RiO
  • Registered mental health Nurse on NMC Register
  • Post-registration experience, evidence of formal courses and training at diploma/degree level
  • Conforms to NMC requirements
  • Registered Mentor
  • Leadership qualification
  • BSc degree in nursing or equivalent level
  • Qualification in providing supervision
  • Qualification in coaching
  • Non – medical Prescriber
  • Psychological intervention Qualification including EMDR, CBT, CBTp, Family Interventions, CAT, DBT
  • Registered Sign off mentor for nurse students
  • Evidence of extensive post registration experience with working with adults experiencing mental health related difficulties
  • Experience of working with people who have difficulty in engaging with services and have complex mental health needs
  • Experience of working in community setting and making decisions autonomously
  • Experience of assessing, planning, co-ordinating and managing people with mental health problems in a community setting
  • Experience of integrating care delivery across multi-disciplinary /multi agency teams
  • Experience of leading/contributing to CPA/SAP process
  • Experience of presenting at local/national level
  • Experience and ability to carry out clinical audit
  • Knowledge of the Mental Health NICE Guidelines
  • Knowledge of NMC Code of Conduct  
  • Understanding of the principles of the Care Programme Approach and Community Care Act
  • Demonstrate understanding of Equality and Anti-discriminatory practice
  • Knowledge of other relevant local and national documents/drivers that affects practice
