Social Worker – Children With Disabilities Team in Devon UK earning £38 Hourly. What you will do:
- Conduct assessments of children with disabilities and their families to ensure they receive the appropriate support and services.
- Develop and implement care plans tailored to the unique needs of each child, promoting their well-being and development.
- Work collaboratively with multi-agency teams, including health, education, and voluntary agencies, to provide consistent and comprehensive support.
- Provide guidance and support to children with disabilities and their families, helping them navigate challenges and access resources.
- Maintain accurate records and documentation in compliance with legislative and organisational standards.
- Participate in duty days at the office and conduct visits across Devon to ensure children receive regular support and care.
