What you will do:
- Conduct assessments and develop care plans tailored to the needs of children with disabilities
- Provide dedicated support to children and their families, ensuring their voices are heard and respected
- Collaborate with multi-disciplinary teams to coordinate comprehensive care
- Facilitate best interest meetings in line with the Mental Capacity Act
- Offer advice and guidance on accessing services and resources
- Maintain accurate and detailed records of interactions and interventions
- Advocate for children’s rights and well-being throughout all interactions and decisions
