Experienced Social Worker – Adult Area Team, Birmingham – £40,476 Annually

What you will do:
- Conduct thorough assessments to identify the needs and strengths of adults within the community.
- Develop and implement robust support plans that promote recovery and independence.
- Provide intervention services in collaboration with healthcare professionals and the Herefordshire & Worcestershire Health and Care NHS Trust.
- Employ the Three Conversations approach to focus on individuals’ strengths and community assets.
- Carry out safeguarding enquiries to protect vulnerable adults at risk.
- Regularly evaluate support plans to ensure they remain effective and suitable.
- Maintain detailed and accurate records while complying with relevant legislation such as the Care Act, Mental Capacity Act, and Mental Health Act.
- Participate actively in team meetings and supervision sessions, ensuring best practice and continuous professional development.
