What you will do:

  • Conduct thorough assessments to identify the needs and strengths of adults within the community.
  • Develop and implement robust support plans that promote recovery and independence.
  • Provide intervention services in collaboration with healthcare professionals and the Herefordshire & Worcestershire Health and Care NHS Trust.
  • Employ the Three Conversations approach to focus on individuals’ strengths and community assets.
  • Carry out safeguarding enquiries to protect vulnerable adults at risk.
  • Regularly evaluate support plans to ensure they remain effective and suitable.
  • Maintain detailed and accurate records while complying with relevant legislation such as the Care Act, Mental Capacity Act, and Mental Health Act.
  • Participate actively in team meetings and supervision sessions, ensuring best practice and continuous professional development.
