What you will do:
– Conduct assessments and develop personalised care plans for adults with learning disabilities.
– Work closely with individuals from ages 18-25 to support their transition to adult services.
– Engage in collaborative work with carers, families, and other professionals to ensure holistic care.
– Provide legally literate guidance and support while understanding the nuances of DoLS and MCA.
– Utilise strong strength-based assessment skills to empower clients in their care journey.
– Maintain accurate and comprehensive records of all interactions and progress.
– Participate in team meetings to develop and adapt strategies for individual care plans.
