Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work full-time in an Adults Single Point Duty Team based in Merthyr Tydfil. The ideal candidate will possess highly developed specialist knowledge of social work practice in Adults' Services and be registered with Social Work England/CCW.

The pay rate for this locum Social Worker job is £36 per hour (LTD Company Equivalent) and is a 3-month ongoing contract.

Purpose of the job:

  • To safeguard vulnerable adults in need of support who are referred to the department, ensuring that plans are put in place which will mitigate risks.

  • To obtain an accurate, person centred perspective of the adults needs in order to ensure that the most appropriate support is provided to achieve their optimal independence.

  • To work collaboratively with Adults and their carers who are experiencing particularly complex issues and assist them in finding solutions.

  • To look preventative Services in the Community and utilise the resources appropriately.

  • To liaise and engage with Cluster Services that is set up in the community.

Main Duties/Responsibilities:

  • To deliver outcome focussed assessments and person centred care plans, which identify the needs of individuals in order to identify the most appropriate support options.

  • To assess risks to service users ensuring that where risks are identified, that Risk Management Plans are in place to mitigate those risks.

  • To undertake Non Criminal Investigations as part of the Safeguarding Adults process .

  • To undertake Best Interest Assessments .

  • To undertake Deprivation of Liberty Assessments .

  • To support adults with complex care needs to maximise their potential .

  • To liaise with Cluster Services ensuring that the clients wellbeing can be met, supported, and managed with a range of preventative services and resources to address their assessed needs.

  • To assist and support the front-line staff to improve the overall quality in the way in which we deliver front-line .

  • Services ensuring citizens receive adequate services at the appropriate time thus preventing long term care and hospital admissions.

Requirements of the Social Worker:

  • Degree or equivalent in social work

  • Current Social Work England / CCW registration

  • Significant senior level frontline experience within Adults' Services

  • Must be a car driver
