Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work full-time within an Adults Reviewing Team based in Dudley. The ideal candidate will possess highly developed specialist knowledge of social work practice in adults' services and be registered with the HCPC.
The pay rate for this locum Social Worker job is £27.50 per hour (LTD Company Equivalent) and is an initial 3-month contract.
Main Duties/Responsibilities:
- To work as part of our Reviewing Team undertaking the review and transfer of packages of care from rehabilitation provision to external providers.
- You will be required to attend a panel to submit your cases for funding agreement. This will involve assessment of the needs of individuals and their carers, working jointly with other professionals; to meet the identified need creatively through universal support and the provision of individualised long term care.
Requirements of the Social Worker:
- Degree or equivalent in social work
- Current HCPC registration
- Significant senior level frontline experience within adults' services
- Must be a car driver
- Direct experience of working with adults, especially those from different racial & cultural backgrounds.
- Knowledge of relevant statutory social work legislation.
- Knowledge of the issues relating to the well-being of service users covering a wide range of clients Ability to prioritise and manage casework, communicate effectively with client groups & professionals in order to assess client needs.
- Ability to undertake person-centred assessments
- Develop care plans and panel reports for both domiciliary care and residential/nursing placements.
- Ability to deal with conflict and complaints. Ability to participate within a duty team framework.
