What you will do:
- Collaborate with health colleagues to minimize hospital admissions, ensuring that individuals in the community receive the support they need in a timely manner.
- Conduct on-site visits as required, including same-day responses to urgent situations, providing a rapid and effective social work response.
- Carry out Care Act and short-term assessments with a focus on promoting independence and reducing the need for ongoing support.
- Provide expert social work input within a dynamic multi-disciplinary team, contributing your specialised knowledge to achieve the best outcomes for individuals.
- This role is primarily office-based but includes travel, so a clean driving licence and reliable transport are essential.
