Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work full-time across North Manchester in the Adults Integrated Discharge Team. The ideal candidate will possess highly developed specialist knowledge of social work practice in Adults' Services and be registered with Social Work England. Weekend working on a rolling rota -daily hours of work 8:45 - 17:00.
The pay rate for this locum Social Worker job is £21.76 per hour (Assignment rate).
Requirements of the Social Worker:
- Degree or equivalent in social work.
- Current Social Work England registration.
- Significant senior level frontline experience within Adults' Services.
- Must have previous experience of hospital discharges and able to work in a fast-paced environment.
- Level 2 knowledge and experience of the care act - mental capacity act - safeguarding - DOLS - housing support and pathways from hospital.
- Must have a working knowledge of the following legislation in relation to hospital discharges: The Care Act 2014, The Care and Support (Charging and Assessment of Resources) Regulations 2014.
