Sanctuary Personnel, an innovative and committed recruitment agency is currently looking for a dedicated and experienced Social Worker to work based in Durham, within the Adults Hospital Discharge Team.
The pay rate for this locum Social Worker role is £184.75 per day (Assignment rate) and is a 3-month contract.
Main Duties:
- Ensure that the social care needs of people are fully assessed and that services are delivered, monitored, and reviewed in accordance with the agreed care plan.
- Undertake assessments of the needs of potential users and obtain contributions to that assessment from other personal and/or agencies.
- Define service requirements and design individually tailored packages of care to meet assessed needs.
Requirements of the Social Worker:
- Degree or equivalent in social work.
- Current Social Work England registration.
- Recent post-qualifying experience within frontline adult social work.
- Expected to undertake Practice Teacher training and take a student after completing 2 years at post-qualifying level.
- Ability to form relationships with users and carers.
- Assessment and care planning.
- Report writing.
- Specific therapeutic skills.
- Ability to work under pressure.
- Ability to work as a member of a team.
- Access to a car or access to a means of mobility support to meet demands for domiciliary visits (if driving, must have a current valid driving licence and appropriate insurance).
