Sanctuary Social Care is currently looking for an experienced and dedicated Social Worker to work full-time within an Adults Hospital Discharge Team based in Southwark. The ideal candidate will possess highly developed specialist knowledge of social work practice in Adults' Services and be registered with Social Work England.

The pay rate for this locum Social Worker job is £27.27 per hour (Assignment rate) and is an initial 3-month contract.

Job Summary:

  • The main function of the team is to support the discharge of those service users whose care needs have changed due to their admission into hospital. This will involve assessing for new packages of care, increasing existing generic care package, supporting with transition from hospital into residential or nursing home care and completing screening assessments for those with rehab and reablement needs.

  • Team members hold their own caseloads but also participate in duty social work rota approximately 2-3 times per month.

  • The team also complete Mental Capacity Assessments and Best Interest planning in addition to overseeing and leading on safeguarding enquiries in situations where the concern has been raised during admission.

Requirements of the Social Worker:

  • Degree or equivalent in social work

  • Current Social Work England registration

  • Significant senior level frontline experience within Adults' Services

  • Good risk management skills and an ability to build positive relationships in truncated time period is also important. Strong communication skills are essential also.

  • Knowledge/experience within:Mental Capacity Assessments, Safeguarding Adults enquiries.

  • Strong assessment and discharge planning skills.

  • Good communication skills.

  • Ability to work and manage need and risk at a fast pace.

  • Strong ability to adapt to different discharge pathways and processes.

  • Good team work ethic.
