Sanctuary Personnel, a leading and award-winning recruitment agency is currently looking for an experienced and dedicated Social Worker to work full-time based in Bolton, within the Adults Community Mental Health Team.
The pay rate for this locum Social Worker role is £26.00 per hour (Assignment rate) and is a 3-month contract.
The Bolton Mental Health and Social Care Directorate is an Integrated service that strives to deliver a needs-based service rather than one based on age. There are two Functional CMHT’s in Bolton that provide a comprehensive multi-disciplinary service to people aged 16 years and above including an assertive outreach approach. The social worker would join the North Functional CMHT in Bolton, which provides a comprehensive multi-disciplinary service to people aged 16 years and above including an assertive outreach approach.
Duties:
Assess service users using identified assessments, risk assessments and undertake the role of Care Coordinator in line with CPA process.
Requirements of the Social Worker:
Work effectively as a member of a multi-disciplinary team, working proactively with defined, complex caseloads which could consist of people who are on the Assertive Outreach pathway.
Be flexible, creative, committed and able to adapt positively to change.
Be able to work effectively, taking a holistic approach to assessment and treatment working in partnership with service users and their carer’s promoting independence and recovery of people with serious and enduring mental health difficulties.
Degree or equivalent in social work
Current Social Work England registration
Previous experience within a frontline social work adult community mental health team
